2020 Quality Recognition Program
Cityview has received the 2020 Quality Initiative Recognition Program by the American Health Association and the National Center for Assited Living for their commitment to improving the lives of the individuals they serve.
Chronic Care Managment
Our Chronic Care Management Program is designed to reduce return-to-hospital incidences by capturing the onset of chronic illness exacerbation and treating our residents in-house. Our Nurse Practitioner is available in-house 5 days a week to coordinate care with our team of physicians and nurses while providing the highest quality of care at bedside.
With more than 50 years of combined experience, our interdisciplinary team is made up of healthcare professionals from various fields working together toward a common goal for our residents.
Physician Integration Model
In addition to our Nurse Practitioner and team of nurses, we have brought onboard a team of specialty physicians as part of our Physician Integration Model. Our facility also provides high-observation rooms for those residents who require increased monitoring and care.
