Patient:  Carol Bruns

Diagnosis:  Right ankle Sprain

Physician:  Dr. Kris Brickman

Length of Stay: (Therapy) 11/10/18 – 1/9/19

Ms. Bruns is a Long-term resident at this facility and the Vice President of our Resident Council. Ms. Bruns goals were to walk without pain due to a right ankle sprain. Prior to the ankle sprain she was able to walk around the facility frequently and attend various activities with supervision without the need for an adaptive device.  After the ankle sprain Ms. Bruns required increased time with ambulation, shorter distances due to pain, increased caregiver assistance and decreased time out of her room.  Ms. Bruns worked with therapy diligently 3 times a week and was trained on Home exercise programs that she was responsible for on her days off of therapy.  Upon completion of therapy Ms. Brun’s ankle strength and Range of motion returned to normal, her distance in walking returned to supervision with in the facility without pain.  She is attending activities and socializing daily out of her room and reports that her ankle is pain free and she continues to do her exercises.

Diagnosis:  Syncope, Huntington’s Disease, Multiple Sclerosis, Muscle Weakness

Length of Stay: (Therapy) 3/11/19 – 4/12/19

Janet Swiczkowski, short term stay resident, who joined the Certus Healthcare family in March 2019.  Upon admission, Ms. Swiczkowski required maximum assistance with self-care and walking, she also required maximum assistance for memory, comprehension and making her needs known.  She required the use of a wheelchair where she needed assistance to propel.  Ms. Swiczkowski’s goal was to return to her previous living arrangement where she was required to be Independent with all self-care, walk with a walker and complete home management tasks.  Janet worked very hard with therapy and was able to achieve several goals and discharge to an assisted Living Facility.  Ms. Swiczkowski was able to complete simple microwavable meals, complete all aspects of her self-care and walk independently with a Rollator.

Patient:  Maxie Sellers

Diagnosis:  Acute and Chronic Respiratory Failure, Weakness

Physician:  Dr. Kris Brickman

Length of Stay:  12/27 - Present

Mr. Sellers is our Regional Director of Nursing’s Father and this is his second stay with us.  Mr. Sellers goals are to return home with his wife with the ability to walk with his walker with confidence around the house and in the community.  Upon admission, he was unsteady with his gait, unsteady standing balance and poor activity tolerance with self-care tasks and mobility.  Mr. Sellers has had medical complications extending his stay, but he has worked very hard on his own and with Speech, Occupational and Physical therapy to meet his goals. He has a strong family support as well as caregiver support within the facility.  Mr. Sellers is continuing Occupational and Physical Therapy to safely return home and has met his Speech Therapy goals.  He is dedicated to his progress and can walk longer distances and require less help with self-care.

Diagnosis:  Altered Mental Status, Muscle Weakness

Shelley Robinson, short term stay resident, who joined the Certus Healthcare family in November of 2018.  Upon admission, Ms. Robinson required maximum assistance with self-care and walking, she also required maximum assistance for memory, comprehension and making her needs known.  She required the use of a wheelchair where she needed maximum assistance to propel.  Ms. Robinsons’ goal was to return to her previous living arrangement where she was required to be Independent with all self-care, walk with a walker and complete home management tasks.  Shelley worked very hard with therapy and was able to achieve all her goals and discharge to intended placement.  Ms. Robinson was able to cook meals consistently, complete all aspects of her self-care and walk independently with a Rollator.
